Challenges such as physician multitasking, delegation of physician roles to non-physicians, and lack of participation can hinder effective hospice care and documentation.<br /><br />A case study of a patient with heart failure and COPD illustrated how comprehensive symptom management and proactive planning in IDG meetings can prevent hospitalizations and support quality care. The presenters also discussed quality metrics like hospice visits in the last days of life and the Hospice Care Index, emphasizing the physician’s role in influencing these outcomes.<br /><br />Finally, the webinar addressed deprescribing practices, encouraging medication reviews to optimize patient comfort and reduce unnecessary treatments while ensuring proper documentation. Questions from attendees covered documentation requirements, team engagement strategies, diagnosis coding, and inpatient hospice care.<br /><br />The session concluded with an invitation to the upcoming Hospice Physician Compliance Conference for deeper education on hospice regulations and physician roles. Overall, the presentation reinforced the hospice physician as an essential leader in interdisciplinary collaboration to enhance patient-centered hospice care.
