Video Summary
The webinar opened with announcements from HMDCB and sponsors, then shifted to a practical discussion on hospice admissions, eligibility, relatedness, and deprescribing. Faculty from Gentiva Hospice reviewed the current hospice landscape, noting increased hospice use, rising regulatory scrutiny, more audits, and the importance of strong documentation to support eligibility and terminal prognosis.

A major theme was that hospice eligibility is not just about decline; patients must have a prognosis of six months or less if the illness runs its normal course and must be accepting of the hospice philosophy. The presenters outlined four pathways to eligibility: a clear fit with LCD criteria, a close fit with rapid decline, decline plus significant comorbidities, and physician clinical judgment. They emphasized that clinicians must “show their work” in the chart, much like solving a math problem.

Several case discussions illustrated how to think through complex admissions, including advanced dementia, severe weight loss, arthritis, poor intake, and uncertainty about a principal diagnosis. The panel discussed how diagnoses such as protein-calorie malnutrition, dysphagia, arthritis, vascular disease, or frailty may support eligibility depending on the clinical picture.

The second half focused on relatedness and deprescribing. The speakers reviewed how to determine whether conditions and medications are related to the terminal diagnosis, and when medications like statins, anticoagulants, antihypertensives, and dementia drugs should be continued or stopped. They recommended using tools like NHPCO guidance and Deprescribing.org, and stressed the importance of careful family communication.

Overall, the session was an interactive, clinically practical review of hospice admission decision-making, documentation, and medication management at end of life.
